This report on systems of care working with long-term foster care youth transitioning to adulthood is the product of a collaboration between Hennepin County and the University of Minnesota, known as the Hennepin-University Partnership, or HUP. It is a comprehensive literature review, annotated bibliography, and a brief guide to current evidence-based practices.
“Systems of care” refers to frameworks for guiding processes and activities through collaborative efforts of multiple systems (both formal and informal) designed to meet the needs of children and families.
